Building a Foundation for a Resilient Future
Our re-housing team is here to help you navigate the path to stable housing and build a stronger tomorrow.
Finding a stable home is a critical step toward rebuilding your life, but the journey can be filled with challenges. The Breakthrough Re-housing Program is designed to empower adults diagnosed with severe mental illness and experiencing homelessness. We’re here to provide the tools and support needed to overcome barriers and secure a place to call home. We believe in your strength and are here to partner with you every step of the way.
Our dedicated team offers personalized support to help you navigate the housing system with confidence. We provide practical assistance that empowers you to move forward.
This includes helping with:
- Connecting with landlords and apartment complexes
- Understanding and completing rental applications
- Securing necessary identification documents
- Finding essential items like a bed for your new home
- Arranging transportation for important appointments
Long-Term Stability and Support
Our commitment to you doesn’t end once you have the keys. Securing housing is just the beginning. We continue to work alongside you, offering ongoing peer support and guidance to help you maintain your new home and build a foundation for lasting stability. By connecting with the U.S. Department of Housing and Urban Development (HUD) Continuum of Care, we ensure you have access to a network of community resources to support your continued success.
