Assisting Homeless Adults Navigate Re-housing
Our re-housing team is ready to assist you when you need it most.
The Breakthrough Re-housing Program is designed to assist homeless adults in overcoming barriers so they can secure housing. This often involves a set of complex circumstances, and help to navigate the system for housing is crucial.
Support often includes inquiries with the following:
- Apartment complexes and landlords
- Explaining a rental application
- Collecting or re-applying for identification documents
- Finding a donated bed
- Transportation to an important appointment
- Ongoing peer support and more.
After housing is secured, the team continues working with the individual to maintain that stable home. The team is connected to the U.S. Department of Housing and Urban Development (HUD) Continuum of Care to access community resources first and then other sources of support, as needed.
